UPDATE 4:50PM: Picton Road remains closed following a serious crash earlier today, and diversions are in place.
A NSW Police spokesperson said about 11.55am emergency services were called to Picton Road, near Mount Keira Road, Cataract to a multi-vehicle crash, including a B-Double truck.
Fire and Rescue NSW firefighters freed a 38-year-old male trapped in a vehicle.
NSW Ambulance treated him for pelvic, chest and arm injuries before he was flown to Liverpool Hospital in a serious condition.
Three people were taken by ambulance to Wollongong Hospital in stable condition.
Officers attached to Wollongong Police District are investigating the circumstances surrounding the crash.

1PM, WEDNESDAY 7 JANUARY: Four people are being taken to hospital after a crash on Picton Road injured 13 people.
The road remains closed between Mount Keira Road and the M1 following the accident, which occurred at 1 pm this afternoon (7 January).
Drivers are urged to avoid the area; light vehicles are directed to use Mount Keira Rd.
Heavy vehicles must park and wait or use Appin Rd and Narellan Rd.

A NSW Ambulance spokesperson said a truck collided with about six cars. The truck driver was initially trapped but later freed from his vehicle uninjured.
Another driver, a man in his 30s, is in a serious condition with pelvis, chest and arm injuries.
He is being treated by the Toll Helicopter medical team and the spokesperson said they anticipate he will be flown to a Sydney Hospital.
Another three patients have been taken to Wollongong Hospital by ambulance in stable conditions.
Ongoing police investigations are expected to take some time.
